Below are two forms we use to begin your counseling experience. The
first is the Personal Data Inventory and the second is our Professional
Services Agreement.
We have provided the Personal Data Inventory for you to print out and
complete prior to your first visit, if you choose. (Please complete one
form for each person attending the counseling sessions.) Otherwise, we
will ask you to fill it out when you arrive.
The Professional Services Agreement provides you with information on
what to expect in our counseling process. You do not need to print this
out as we will discuss it at the beginning our first meeting.
